How to Enroll/New Enrollment Information

When you are eligible for Medicaid, you need to choose a health plan. The Louisiana Department of Health and Hospitals (DHH) works with Maximus, an enrollment broker, to enroll members in a BAYOU HEALTH plan.

The Enrollment Specialists can:

Help you and your family enroll in a health plan
Answer questions about your choice of health plans
Help you if you decide to change health plans

They have information about LaCare and can answer your questions. You can talk to an Enrollment Specialist by calling 1-855-229-6848 (TTY 1-855-526-3346). You can also enroll by logging onto www.bayouhealth.com.

Once you are enrolled in LaCare, you will get a Welcome Kit. The information in this kit tells you about all of your benefits and services.

Please Note: Once you are enrolled in a plan, you will have 90 days from the postmark date on the Notice of Enrollment to change plans (disenroll). You can change plans for any reason. If you choose not to change plans in this 90-day period, you will be locked into that plan for 12 months from the postmark date on the Notice of Enrollment or until the next annual open enrollment period.

There are situations in which you are allowed to change plans during the lock-in period. If you have any questions about this, please call Member Services.
